Regenerative plantar fasciitis treatments: Support healing and reduce the need for surgery
Plantar fasciitis is irritation and microtearing of the thick band of tissue supporting your arch from heel to toes, often causing sharp “first step” heel pain in the morning. Non surgical treatments are included in standard care. Non surgical treatment aims to reduce the load on fascia and calm the inflammation. This includes calf and heel fascia stretching, custom orthotics and heel cushions to support the arch while offloading the heel, night splints that maintain gentle dorsiflexion overnight, structured physical therapy using manual care, progressive loading and short term corticosteroid treatments. The majority of cases are resolved without surgery. However, chronic or recalcitrant conditions can persist because of ongoing overload, poor tissue quality or biomechanical problems. In severe cases of refractory tensile loads, surgical care, most commonly partial plantar facia release, can reduce the tensile force. However, it comes with risks, such as arch instabilities, nerve irritation and a prolonged recovery.
Regenerative therapies complement standard care by enhancing the fascia’s healing environment. The Institute offers Peptide Therapy, Platelet Rich Plasma (PRP)/Prolotherapy, and Active Cell Therapy (stem cell–based). Peptide therapy modifies inflammation, and increases organized collagen syntheses to improve early healing. It also reduces downtime. PRP/Prolotherapy promotes robust fascia repair by stimulating the local healing response. Active Cell Therapy stimulates progenitor cells and bioactive signals to promote durable regeneration. This can improve tissue quality, reduce the need for surgery, and provide longer pain relief than corticosteroids.
Regenerative care can accelerate healing by utilizing biologic signaling, scaffolds and improving outcomes. It also reduces the need for surgery because it improves tissue quality and heals faster. Other advantages include minimal invasiveness, personalization, based on symptoms and biomechanics and activity goals.
The diagnosis relies on the clinical evaluation as well as imaging, where indicated. Ultrasound is used to assess fascia structure and thickness; X rays are used to evaluate spurs or exclude bony sources; and MRI can be performed for atypical and refractory symptoms. The Institute employs ultrasound guidance for precise biologic delivery and uses a patient centered, evidence based approach to loading progressions, footwear optimization, and rehab. Expect an initial assessment of history, exam, biomechanics, and imaging to confirm stage and plan care; image guided biologic treatments when appropriate; and follow up to track pain, load tolerance, and function with a tailored return to activity plan.
